Making a complaint

Have you witnessed an event that threatened scuba diving safety or a situation deemed unacceptable? Please let us know by entering the information in the appropriate form, the analysis of which will better guide actions to reduce the risk of scuba diving accidents. Complaints and comments management process

To report a problem related to the training and evaluation relating to the qualification in recreational underwater diving from the Government of Quebec, please use the form entitled "Complaint form – accreditation commission". For any other event or situation to report, please use the “General Complaint Form”. An acknowledgment of receipt will be sent to you and the complaint will then be treated as soon as possible and in a confidential manner. Any complaint concerning the evaluation of representative will be dealt with by the accreditation commission within a maximum period of 30 days, in accordance with the policy in force.

How to complete the form

In all cases, give the answer that seems most correct to you and report in your own words what you have witnessed. The recounting of the facts is important for the understanding of the events. Please do not report hearsay. If you don't know the answer to a question, don't answer and move on to the next one.
